Taking Eye Care to Another Level

Leading an established market leader in refractive surgeries is no easy task but for a determined and visionary young woman, it is a challenge worth taking. Sandy Tan joined Optimax Eye Specialist more than two years ago and today, she is leading the organisation as its Chief Executive Officer with exciting plans to further enhance the Optimax brand.

INPG55-02.pngShe is a law graduate and was also an image consultant in the fashion industry. Today, she is leading Optimax Eye Specialist to greater heights with its own full fledged specialists hospital in Kuala Lumpur that is coming up in a few years’ time.

It could be said that Sandy Tan was born into a world of privilege as the eldest daughter of Optimax founder Dato’ Tan Boon Hock but the position she held today speaks more of her capabilities in leading the organisation than her station in life. The eloquent 30-year-old has big dreams for the company and she is diligently working her way towards achieving it.

“In 10 years, we will be a full fledged hospital providing health care with the latest technology and the best in customer care,” she said of her vision for Optimax Eye Specialist. Even now, plans are underway to construct an Optimax Eye Hospital in Kuala Lumpur, just 10 minutes away from the headquarters in Mont Kiara.

Sandy first started her full time career in Optimax more than two years ago after brief stints in banking and the fashion industry. She first gained experience as a Capital Market and Equity Department Executive before becoming an image consultant and later, a buyer at homegrown fashion label British India where she honed an acute and irrefutable sense of marketing.

She joined Optimax as the Head of Marketing and this is one decision that she does not regret as she found it an exciting experience that sets the pace for her to assume her current role as the Chief Executive Officer. “I grew up learning about Optimax from my father when he came home and talked about work, so working here is an easy transition from my previous jobs,” she said.

She does not see her young age as a deterrent in fulfilling the heavy responsibility of leading Optimax but instead sees it as an advantage as she could work out marketing and branding that appealed to the younger generation. “Optimax has been around for 21 years so it is time to make it trendier now to appeal to the younger generation,” she said.

This was how the cute round-eyed blue Opti – the Optimax mascot – was introduced last year. It is one of the marketing methods, along with a comprehensive social media presence, introduced to push the Optimax brand further amongst the young and old.

Optimax today is a leading eye care specialist centre that provided eye care services from eye examination to laser vision correction. It is ISO certified and it is the recipient of Frost and Sullivan Award under the Eye Care Centre (Lasik) category. The eye care centre has served 100,000 satisfied patients to date with its team of in-house engineers and doctors specialising in vitreoretinal, cornea treatment and paediatrics.

Optimax, which adopts the Vision For Life programme and offers a lifetime warranty for patients, now has a private eye specialist hospital in Penang with two more centres in Sri Petaling and Ipoh.

However, it is not all about dollars and cents for Sandy when it comes to running Optimax. Corporate Social Responsibility is as important as growing the brand. “We do a lot of charitable programmes and we are starting a foundation to do Lasik for the poor,” she said. Optimax has also provided over 200 free cataract operations for poor patients. Optimax is also one of the sponsors of the WAKE program hosted by Agape Vision and regularly offers free eye checks to residents of various orphanages and old folks homes in the country.

Also, there are plans underway to collect preloved glasses disposed by Optimax clients and donate these glasses to orphanages and old folks homes in Penang. “Some of our clients who have their vision corrected do not need glasses anymore so we will collect all these glasses, change the lenses and donate it to children and senior citizens in those homes,” she said.

It is without a doubt that under Sandy’s leadership, Optimax Eye Specialist is heading towards a bright future as a company with a heart and an eye for the future.

Debunking Breastfeeding Myths

“While breastfeeding may not seem the right choice for every parent, it is the best choice for baby. – Amy Spangler”

There is plenty of helpful information about breastfeeding as well as plenty of misinformation that would hinder successful breastfeeding. Below are some common myths that need to be corrected.

Myth 1 – Breastfeeding will come naturally

Breastfeeding is natural and normal but not necessarily easy! 92% of new mothers had at least one concern on the third day of breastfeeding (eg. poorlatching, low milk supply, breast pain) (Pediatrics 2013). Unfortunately, most mothers are discharged 2 days after delivery and the period immediately after delivery is the most challenging for breastfeeding. Mothers can get help from a breastfeeding support group or a lactation consultant.

Myth 2 – Pain is normal

Breastfeeding is not always painful. There may be an initial tingling sensation but if baby is properly latched, it should not hurt. Mothers are taught holding positions for bottle feeding. Try reclining with the baby’s weight against the mother’s body.

Myth 3 – Need to top up as your milk supply is low

For the first 3 to 5 days after delivery, mothers do not produce much milk but they produce a thick concentration of liquid (colostrum) that the newborn needs. At birth, the fullterm newborn’s stomach is the size of a marble (5 ml) and grows gradually in a week to the size of a chicken’s egg (20ml).

As a lactating mother’s breast is always making milk, thus the emptier the breast, the faster milk is produced to replace it. If a mother consistently waits until her breasts ‘fill up’ before she nurses, her body may get the message that it is making too much milk and may reduce the total production.

Myth 4 – Your new born will feed every two to three hours with each feed lasting for 30 minutes

Frequency of feeding will vary according to the mother’s milk supply, storage capacity and baby’s developmental needs. Growth spurts and illnesses can temporarily change baby’s feeding patterns.

During breastfeeding, the initial portion of the breastmilk (foremilk) has a lower caloric level compared to the latter portion of the breast milk (hindmilk), which is higher in fat content. Hence, putting arbitrary limits on frequency and duration of breastfeeding may lead to inadequate caloric intake. In other words, milk production has been shown to be related to feeding frequency, and milk supply declines when feedings are infrequent or restricted.

Myth 5 – Never wake a sleeping baby

Newborns should be awakened to drink, if necessary, at least 8 times a day. In fact, newborns who experience early skin-to-skin* contact immediately after birth for at least one hour (‘The Golden Hour’) are more likely to latch on well, especially if the mother did not receive medication during the labour or birth. It has been shown that separation, results in delayed response to feeding; exhibiting shutdown behaviour which in turn can affect feeding. (*baby naked, not wrapped up in blanket and place on mother’s chest or abdomen)

Myth 6 – A bottle or pacifier can create

nipple confusion

It is not so much ‘confusion’ but ‘preference’! Even though it would be easier to suck milk from a bottle compared to suckling from the breast, early introduction of a bottle or pacifier before breastfeeding is well-established is discouraged as breast and bottle feeding require different oral motor skills. Rubber or silicon nipples provide a type of ‘super stimulus’ that can result in inappropriate sucking technique on the breast when babies switch between breast and bottle.

Myth 7 – You cannot breastfeed if you had

Women with breast implants have been able to lactate successfully especially if incisions were made on the underside of the breast. Women with breast reduction may have more difficulty especially if the nerve endings around the nipple have been cut. (American Soc. of Plastic Surgeon, 2011)

Myth 8 – Breastfeeding helps to lose weight

Breastfeeding helps to lose 300-500 calories per day but your body needs energy (read: calories) to create breast milk. It is recommended that a few hundred extra calories are taken during the breastfeeding period.

Breastfeeding mothers may actually gain weight as many of them commonly experience ravenous hunger while feeding. Together with hormonal (prolactin) changes it can lead them to hang on to pregnancy weight or gain more weight. Hence, mothers should try improving the quality of the food and snacks and keeping up with fluid intake for weight control.

Myth 9 – Women who breastfeed get less sleep

Sleep deprivation is an inevitable part of having a baby which begins during pregnancy, when sleep needs may increase due to discomfort and frequent trips to the bathroom which interfere with a full night’s sleep. This is compounded in the postpartum period.

Studies have shown that not only do breastfeeding mothers get more sleep, but the sleep they get is of better quality when baby is in proximity to the mother. It was shown that breastfeeding mothers who bedshare got the most sleep. Mothers who were bottle feeding got the same amount of sleep whether their babies were with them or in another room. (Quillin & Glenn 2004).

Myth 10 – Breastfeeding is birth control

Breast feeding can be an effective birth control ONLY if certain conditions are met: such as baby is younger than 6 months of age, exclusive breastfeeding (no supplemental feeds of water, formula) only, mother has not started having periods. With these factors, the mother has a more than 98% protection against pregnancy.

As soon as there is a decline in breastfeeding, due to the baby eating supplemental feedings or nursing less often, the contraceptive protection decreases.

ROBOTIC SURGERY

There are many types of Robots in medicine today. Some will automatically take your blood pressure as you walk around, automatically inject insulin for diabetic, shock the heart to keep it beating, move your arms or legs and the list is growing.

Robotic surgery is advancing very quickly.  The Robots entered the areas of knee replacement surgery in 2006. One of the new leaders in robotic surgery is StrykerMako. This device allows the replacement of the Knees either fully or partially. The robotic arm technology, (Whole Arm Manipulator “WAM”) was developed by MIT and Barrett Technology.

The human knee of bones, muscles and ligaments are very complex.  The replacement by a metal joint assures the patient that he can walk or run with less pain.  Therefore, computers have been used for years along with x-rays to make sure the length and alignment of the metal knee is accurate.  However greater accuracy appears to be now available with a Robot.

The StykerMako RIO system using a CT x-ray to create 3D model of the patient’s knee assisting the surgeons to develop a plan that prepares the for the implant, the size needed, alignment and positioning.  This customized plan is used to assist during surgery with the added placement of pins in the leg then integrated with the 3D CT and real time views guiding the surgeon during surgery.  The real time information increases the accuracy giving greater safety for an ideal implant positioning which reduces potential complications.

Studies on the MAKOplasty patients using randomized controlled trials show the patients experience less pain days after surgery than those that underwent manual surgery after two months.  Other studies on the partial knee replacement have shown the robot is 2-3 times more accurate than manual techniques.  Interim results of patients indicate the robotic arm has a 0.4% failure rate versus a 4.0% rate for manual implants.

From partial knees in 2006 to full knees in 2015 over 23,000 MAKOplasty procedures have been preformed using the robot.  There are now over 150 hospitals worldwide using the Robotic system. 

As with ALL new medical interventions evaluation over time is the key.  To date with thousands of knees replaced there seems to be an advantage in robotic knee surgery with less pain and quicker return to normal life in the Unicompartmental Knee Arthroplasty (UKA).

WHY WE NEED TO DETOX & LIVE? PART II – SOME GOOD TIPS TO FOLLOW:

• BE A TURKEY ( A COLD ONE) AND TAKE A DRUG HOLIDAY

There is no way to handle a true physiological addition except to stop it completely. Addicts can’t have just on line of cocaine of just one drink.  Go Cold Turkey, but you won’t have to white knuckle it because if you follow – “5 Ways to Detox” you will automatically reset your body’s neurotransmitters and hormones.

Stop all form of sugar, all flour products and all artificial sweeteners.  They all cause increased cravings and slow metabolism and lead to fat storage.  This especially includes liquid sugar calories – A latte can have more sugar than a can of soda- that makes you eat more all day and drive storage of belly fat. Also get rid of anything with Trans or Hydrogenated FATS & MSG (watch hidden names).

Ideally, for 5 Days you avoid any foods that come in a box, package or can or have a label, and stick to real, whole fresh food. And the best way to really Detox is to give up all the grains for 5 days. Give up all drugs too, Caffeine, and alcohol are the 2 biggest after sugar.

Beyond Feeling Blue

Karen is a 38-year-old experienced piano teacher who is very dedicated to her work. She is a talented piano teacher who has won many awards at the competitions. Karen used to think that she should get the award whenever there is a competition. Recently she was not nominated for an outstanding teaching award which she has been long waiting for. Suddenly she thought of her career life is in ruin and it felt as if the sky has fallen. She has withdrawn herself from other people, feeling depressed with a lack of interest in doing anything coupled with lack of motivation to go to work. She felt fatigue most of the day but had difficulty to fall asleep at night. She thought that everything was gone wrong in her life and it was her fault. She was feeling upset and wallowing in self-guilt for not being nominated for the outstanding teaching Award. She had thoughts about suicide as a solution to her unhappiness.

Everyone occasionally feels blue or sad. If one feels depressed for a prolonged period of time and it interferes with one’s daily life and normal functioning, or to the extent of developing physical problems, one is likely to have developed depression.

Depression is caused by a combination of biological, psychological and social factors. Depression is a “whole-body” illness, involving your body, mood, and thoughts. It affects the way you eat and sleep, the way you feel about yourself and the way you think about things. It is not a sign of personal weakness or a condition that can be willed or wished away. People with a depressive illness cannot merely “pull themselves together” and get better.

According to the report issued by the World Health Organization, over 350 million people around the world have depression. Up to 15 per cent of patients suffering from major depressive disorder eventually commit suicide. Depression can be reliably diagnosed and treated in primary care, but in many countries, there is less than 10% of those affected have access to effective treatments.

The frequency, severity and duration of symptoms may vary from person to person. According to American Psychiatric Association, DSM-V, if five (or more) of the following symptoms have been presented for over two weeks; at least one of the symptoms is either depressed mood or loss of interest or pleasure, it is highly possible that one has developed depression:

• Emotion

Markedly diminished interest in almost all activities, depressed mood, feelings of worthlessness or feeling hopeless, sad or empty. (note: in children and adolescents, can be irritable mood)

• Cognition

Recurrent thoughts of death, diminished ability to think or concentrate, or indecisiveness.

• Physical

Insomnia or hypersomnia, fatigue, decrease or increase in appetite, weight loss or weight gain.

• Behaviour

Social withdrawal, lack of motivation or suicide attempt.

Identifying SCOLIOSIS in Children

Many scoliotic spines are detected by those who work closely with children, such as a swimming coach or ballet teacher as they will notice slight changes in posture due to the effects it will have on the child. Unfortunately, Scoliosis is often not detected until it is a significant problem.

Why does this happen?

Scoliosis is a condition in which the spine (backbone) bends to the side abnormally. There are 3 directions in which a spine can move, forwards and backwards, twisting left and right and bending left and right. In Scoliosis, the spine adopts the abnormal bending of spine as its new normal. The abnormal bending of spine then becomes permanent and the bones change shape as the scoliosis enlarges and ages, and become more rigid and permanent.

Scoliosis is often not easily detected early. Scoliosis hides beneath the children’s musculoskeletal framework and can often increase to significant levels with only showing minimum hints that it is there. The importance of recognizing and acting on these hints is immeasurable!

Simple Guideline to identifying Scoliosis at an early stage

The child should be viewed from behind, barefoot with the spine easily seen and the legs must be straight, but relaxed.

  1. Start at the pelvis. If the pelvis is higher on one side, this may be an indication that there is a longer leg on one side.
  2. Look at the waistline (the area between the ribs and the pelvis). Check if there is a deeper crease on one side compared to the other.
  3. The shoulder blades should be mirror images of each other. If one shoulder blade is higher than the other, slightly forward compared to the other or twisted, then a scoliosis may be present.
  4. The shoulder girdle (the set of bones that supports the forelimbs) should be level, or closely level. Most people tend to be slightly lower on the dominant side, but this shouldn’t be more than a few millimeters in difference.
  5. Check the position of the neck if it is straight, tilted or bent.
  6. Lastly, look at the spine and the ribs. The spine should be straight and the ribs should not show any signs of a hump forming.

Test for Scoliosis

There is a test called the “Adams forward bending test”, which can be done after the visual check. Ask the child to bend forwards while standing with legs straight. A raised section on either side of the spine strongly indicates that there could be a scoliosis present.

Why Early Detection of Scoliosis helps?

Early detection of scoliosis helps to ensure that early treatment is applied. As scoliotic spines worsen, the treatment options become lesser and the possibility of positive results after treatment also reduces. All children should be screened for scoliosis at least by the age of 8 and this test should be done yearly even if no abnormality is seen.

Celebrating 20 Years of Excellence

Combat Adult SCOLIOSIS

When we hear about scoliosis, we always think it is only related to children. However, scoliosis is also common in adults.

Types of Scoliosis in Adults

  1. Degenerative Adolescent Idiopathic Scoliosis: The person who had scoliosis as a child and then later develops complications.
  2. De-Novo Scoliosis: Scoliosis that develops spontaneously in adult life and is usually secondary to degenerative changes of either the bone or the disc between the bones.

Symptoms of Scoliosis in adults is usually very different to that of children, with pain being the most common. The pain usually starts as a secondary complication of the scoliosis itself. It usually associated with degenerative changes. Common conditions associated with scoliosis include conditions such as a disc prolapse or spondylosis (spinal degeneration accompanied by pain). Very often the associated conditions on its own may be easily treated, but not when Scoliosis is present.

Common age of patients who present with pain is usually around 35 years. This is the age at which the skeleton reaches full maturity and loses its flexibility, thereby making it less able to adapt to the stresses of the scoliosis. Early pain symptom may alert the person to the underlying scoliosis and then measures can be put in place to prevent the condition from getting worse.

  • Postural Changes

Even if the spine curve of an adult is at a low degree early on, there is still the possibility of progression. This means that a spine curve of 20 degrees as a child, may progress to 35 degrees by the age of 35 as an adult. Spine curve at 20 degrees may not shown any postural changes, however, by the time the spine curve reaches 35 degrees there will be a definite change to the person’s posture.

Treatment options for Scoliosis in Adults

Treatment options for Scoliosis in adults are very similar to that of children with one exception. Surgical treatment for adult is only done as the final option in very selective cases. This is due to a much higher risk of complications and a significantly slower recovery compared to children.

The usual conservative options are physical therapies such as Chiropractic and Physiotherapy for both children and adults.

In more severe cases pain management can be achieved with prescription drugs. Other options include epidural injections, nerve blocks and facet joint injections.

As with children, a back brace may also be required. The brace of choice for adults and children, is the new dynamic soft brace. In children, the main function of the brace is to prevent progression of the curve of the spine. In adults, the main function of the brace is to help relieve the pain. Better postural improvements may be achieved as the secondary benefit of the brace.

The most important thing in both children and adults, is the early detection of Scoliosis, thereby improving the outcome of any treatment and preventing future complications.

What is Detox?

Eliminating wastes and poisons via the lymphatic, digestive and urinary systems. Secondary routes of elimination are via the SKIN and LUNGS. Bad breath or recurrent skin eruptions may be evidence of the lung’s and skin’s attempt to assist overworked of weakened primary organs of elimination.

All the DIGESTIVE ORGANS play a crucial role in detoxification. The LIVER, in particular, is responsible for oxidising, breaking down or otherwise processing toxins. It filters a litre of blood per minute, straining out ENDOTOXINS, BACTERIA and POISONS.

Some of these toxins are deposited into bile, which is squirted into the INTESTINES by the gallbladder and moved out of the body. Prescription drugs, birth control pills and other hormonal medications, high fat diets, gallbladder diseases and variety of other factors can reduce the quantity and the quality of bile produced. Hepatitis, cirrhosis, biliary disease and nutrient deficiencies can all hamper the liver’s ability to effectively purify blood.

The KIDNEYS receive toxins, and the glomeruli, a specialised apparatus within the kidneys, filters the blood and metabolises water, electrolytes and nitrogen based waste. Heavy metals, long term aspirin consumption and numerous pharmaceutical medications can damage the kidneys and hamper their ability to detoxify these substances possibly leading to cancers. Toxemia is the precursor to AGING, loss of tone and function and permanent degeneration.

In FUNCTIONAL MEDICINE, the bowels are considered the soil of the body, Nutrient absorption, metabolism, and synthesis of many hormones and enzymes, as well as processing and elimination of waste products and toxins, all occur in the BOWELS. The enzymes, proteins, fats and hormones found in every cell of the body are largely the result of what the digestive organs absorb, metabolise, secrete and excrete. Like a chain reaction, intestinal constipation will lead to cellular inflammation.

No discussion of detoxification would be completed without mentioning the organs of elimination. Detoxification programs target organs whose role is to cleanse and protect your body from toxic substances.

Primarily, the liver, kidneys, colon, lymphatic and blood system.  Detoxification can target each of these organs individually or all of them together.

What is the difference with other DETOX & INTERGRATED DETOX?

HEALING CRISIS: The most common occurrences during a detoxification programme especially one that focus primary on treating the LIVER without first correcting the environment and the gut is sometimes violent reaction known euphemistical as Healing Crisis. Patients may feel nausea, pain, headache, aggravations of inflammatory symptoms or increased sensitivity to dietary and environmental chemicals. This is caused by stimulation of the Liver detoxification without the correct supplements to support the entire process. This leads to an increase in the production of reactive intermediate harmful substances that causes damage to cells and tissues and increase the production of inflammatory medication.
